To complete registration for the 2024-2025 academic year, please complete the following forms, found linked below. For a paper copy, please contact the office.

• Admission Application packet for Academic Year 2024-2025

• Enroll online in FACTS Tuition Management Program

• $250 non-refundable Registration Fee (include non-refundable $200 Academic and Book Fee)

• Copy of Birth Certificate

• Copy of any Sacramental Certificates, i.e. Baptism and Holy Communion

• Student Medical Records

• Parish Subsidy Form

• Student Internet Acceptable Use Policy

• CSA Media and Contact Release Form

• SMS Student Medical Form

  • Please review the SMS Health Letter for information provided by our Nursing staff

If your child is transferring from another school, please include the following:

• Copy of last report card

• Copy of latest standardized testing

• Copy of IEP or accommodation plan

• Signed release form to access records
